Calcitriol (1,25-dihydroxyvitamin D) is the biologically active form of vitamin D that regulates calcium and phosphorus metabolism. This test measures calcitriol levels in the blood to evaluate vitamin D metabolism and diagnose disorders of calcium homeostasis. Calcitriol is produced in the kidneys from 25-hydroxyvitamin D under the control of parathyroid hormone (PTH) and fibroblast growth factor 23 (FGF-23). It stimulates intestinal absorption of calcium and phosphorus, promotes bone mineralization, and suppresses PTH production. Abnormal calcitriol levels can indicate primary hyperparathyroidism, hypoparathyroidism, renal osteodystrophy, or vitamin D-resistant rickets. This test is particularly valuable for evaluating patients with unexplained calcium disorders, monitoring vitamin D metabolism in kidney disease, and diagnosing rare genetic disorders affecting vitamin D activation.
