Free (nonesterified) fatty acids (NEFA) are the portion of fatty acids in the blood that are immediately available for energy production and metabolic needs. This test measures NEFA levels to evaluate metabolic status and diagnose disorders of fat metabolism. NEFA serve as an important energy source for muscles, heart, brain, and other organs when glucose availability is limited. Elevated NEFA levels can indicate increased fat mobilization due to stress, fasting, diabetes, or hormonal disorders. Prolonged elevation may contribute to insulin resistance, cardiovascular disease, and metabolic syndrome. This test is particularly valuable for evaluating patients with unexplained weight loss, monitoring metabolic disorders, and assessing the response to treatments that affect fat metabolism. The test requires fasting to ensure accurate interpretation.
Patient Prep
Sample should be collected in the early morning after a 12-hour fast. Blood for the NEFA procedure should be collected after an overnight fast because the level of circulating nonesterified fatty acids is strongly influenced by food ingestion. If the patient has not properly fasted, the determined level of NEFA will be elevated and not directly comparable to a normal range derived from fasting normal controls.
