Immunoglobulin M (IgM) is the first antibody produced by the immune system when the body encounters a new infection or pathogen. This test measures IgM levels in the blood to evaluate humoral immunity and help distinguish between acute and chronic infections. IgM antibodies are large molecules that don't cross the placenta, so their presence in newborns indicates congenital infection rather than maternal antibodies. High IgM levels can indicate active infection, certain blood disorders like Waldenström macroglobulinemia, or autoimmune conditions. Low IgM levels may suggest immunodeficiency disorders, making patients more susceptible to infections. IgM levels are particularly useful for evaluating the timing of infections and for diagnosing certain rare blood disorders. This test is commonly ordered when evaluating patients with suspected immune deficiencies or when trying to determine if an infection is recent or ongoing.
